South Park is never shy from referencing Japanese anime and culture in their episodes where one episode made fun of Japanese martial arts tropes while another one paraodied Pokemon. This time, the creators requested their fans to send in yaoi/homoerotic fanworks of Craig and Tweek for the latest episode.
Incredibly, the American animation series is very popular with Japanese, where fanarts network site Pixiv have over 10,000 pictures tagged with it and quite a bit of it is yaoi/R-18 related. Now the creators have taken notice of the adoration and decided to include this in this episode. Senpai really has noticed!
